Mục tiêu: tối ưu hóa quy trình real-time PCR định lượng nồng độ ADN virut BK có độ nhạy cao trong huyết tương, nước tiểu ở bệnh nhân sau ghép thận. Vật liệu và phương pháp: kỹ thuật real-time PCR được phát triển, tối ưu với độ nhạy cao trên cơ sở phân tích trình tự vùng gen đặc hiệu VP1 của các chủng virut BK từ bệnh nhân sau ghép thận ở Việt Nam và đánh giá độ chính xác, độ ổn định trên mẫu chuẩn cũng như đánh giá độ nhạy, độ đặc hiệu trên 30 mẫu nhóm bệnh virut BK (+) và 30 mẫu nhóm chứng người hiến máu tình nguyện virut BK (-). Kết quả và kết luận: quy trình real-time PCR tối ưu có độ chính xác cao khi đánh giá trên các mẫu chuẩn nồng độ 104, 103, 102 (copy/µl) với hệ số biến thiên nội phản ứng lần lượt là 0,42%; 1,92% và 1,07%, hệ số biến thiên liên phản ứng lần lượt là 3,38%; 1,28% và 1,47%, độ ổn định 4 tháng ở điều kiện -20oC. Khi đánh giá trên các mẫu nhóm bệnh và nhóm chứng, kỹ thuật đạt độ nhạy, độ đặc hiệu 100%. Đồng thời, kỹ thuật phù hợp tốt so với kít thương mại Realstar BKV PCR 1.0 (Hãng Altona) với hệ số Kappa 0,73 trong xác định virut BK. Kết quả bước đầu cho thấy, kỹ thuật real-time PCR này sử dụng phù hợp hơn trong chẩn đoán virut BK trên bệnh nhân ghép thận ở Việt Nam, đã định lượng được 8/30 mẫu virut BK (+) có nồng độ 102 - 104 copies/ml mà kít Altona bỏ sót. Như vậy, đã tối ưu thành công quy trình real-time PCR định lượng virut BK, góp phần theo dõi, giám sát virut BK ở bệnh nhân sau ghép thận, giảm thiểu tối đa nguy cơ thải ghép do virut BK có nguy cơ dẫn đến bệnh thận.

Abstract
Objectives: To optimize a novel high sensitive real-time PCR quantitative assay in plasma and urine specimens for BK polyomavirus screening strategies and treatment options in renal allograft recipients. Materials and methods: A real-time PCR assay for BK polyomavirus quantification was developed and optimized based on sequence analysing of VP1 gene of BK virus strains isolated from the Northern Vietnamese renal transplant recipients, and evaluated the reproducibility, stability on standard panels, including 102, 103, 104 copies/µL concentration (recombinant plasmid), as well as the sensitivity, specificity on control groups. Results and conclusion: Intra assay precision of optimized real-time PCR assay was excellent reproducibility based on the linear range of quantification with low CV values which were 0.42%, 1.92% and 1.07% of 104, 103 and 102 standards, respectively. Additionally, a high degree of inter assay precision of the assay was also identified (CV values were 3.38%, 1.28% and 1.47% of 104, 103 and 102 standards, respectively), and the stability of 4 months when stored at -20oC. Both the sensitivity and specificity of our assay were 100% when we evaluated on 30 BK polyomavirus (+) samples and 30 BK polyomavirus (-) samples of volunteer blood donors. Furthermore, a comparison of 30 BK polyomavirus (+) samples showed a good agreement between this assay and RealStar BK polyomavirus PCR 1.0 kit results (Altona Diagnostics, Hamburg, Germany) when analyzed by Cohen's kappa statistic. Notably, 8 other positive BK polyomavirus samples which were quantified by our assay were undetermined by RealStar BKV PCR 1.0 kit. In conclusion, the real-time PCR assay is a reliable non-invasive method for measuring BK polyomavirus load in plasma and urine specimens, and identifying patients at risk of BK virus-associated nephropathy in Vietnamese renal post-transplant patients.
