Mục tiêu: đánh giá thực trạng chấp hành quy trình khử khuẩn, tiệt khuẩn và mức độ khử khuẩn ống nội soi mềm tại Bệnh viện Triều An. Phương pháp: nghiên cứu quan sát 120 lần thực hành quy trình khử khuẩn, tiệt khuẩn bằng tay đối với ống nội soi mềm dùng trong nội soi tiêu hóa của 6 nhân viên y tế trực tiếp thực hiện quy trình. Định lượng vi sinh trên 60 mẫu ống nội soi mềm sau khi được khử khuẩn, tiệt khuẩn để đánh giá mức độ khử khuẩn. Kết quả: tỷ lệ sai sót chung 18,3%. Nhân viên y tế thường sai sót ở bước 1 (tiền làm sạch), bước 2 (tháo ống), bước 3 (kiểm tra rò rỉ) và bước 7 (tráng và làm khô) với tỷ lệ lần lượt là 0,8%; 5,8%; 10,8% và 10,8%. Xét nghiệm các mẫu ống nội soi mềm nuôi cấy vi sinh cho thấy có 8 mẫu không đạt (13.3%), trong đó 03 mẫu (37,5%) mọc từ 5 khóm vi sinh trở lên. Phân tích tương quan cho thấy mẫu vi sinh không đạt trong nhóm đối tượng có sai sót cao hơn trong đối tượng không có sai sót trong thực hành khử khuẩn, tiệt khuẩn. Kết luận: chấp hành quy trình khử khuẩn, tiệt khuẩn không đúng làm gia tăng mức độ nhiễm khuẩn ống nội soi mềm.

Abstract
Objectives: To investigate the reality of execution for disinfection and sterilization procedures and disinfection levels of the gastrointestinal flexible endoscopy at Trieu An Hospital. Methods: 120 observations in disinfection and sterilization processing for gastrointestinal endoscopy of all 6 technicians were conducted underling the guideline from the Ministry of Health. Microbiology tests were conducted with 60 samples of flexible gastrointestinal endoscopes. Results: Overall error rate for implementation process was 18.3%. There were four steps at which the technician made frequent mistakes: Step 1 (pre-cleaning), step 2 (tube removing), step 3 (leakage checking) and step 7 (rinsing and drying) with the corresponding rates of 0.8%; 5.8%; 10.8% and 10.8%, respectively. Bacteria was found in 8 samples (13.3%), of which 03 samples appeared more than 5 microbiology groups (37.5%). The percentage of bacteria-detected-samples in incorrect perfromances was significantly higher than that in the standard group. Conclusion: The errors in disinfection and sterilization procedure might increase infection levels of the gastrointestinal endoscopy.
