Tóm tắt
Mục tiêu: mô tả thực trạng nhiễm độc chì của người lao động tại làng nghề tái chế nhôm Bình Yên tỉnh Nam Định và tìm hiểu một số yếu tố liên quan. Đối tượng và phương pháp: nghiên cứu mô tả cắt ngang từ 2015 - 2016. 418 người lao động từ 18 - 60 tuổi được lựa chọn ngẫu nhiên để tham gia nghiên cứu. Sử dụng phương pháp phổ hấp thu nguyên tử bằng lò grafit (GFAAS) để xác định hàm lượng chì trong máu của người lao động. Đánh giá tình trạng nhiễm độc theo Thông tư 15/2016/TT-BYT ngày 15/5/2016 của Bộ Y tế. Kết quả: thực trạng nhiễm độc chì: 88,3% người lao động có hàm lượng chì máu < 40 µg/dl (mức an toàn); 11,4% người lao động có hàm lượng chì máu từ 40 - < 80 µg/dl (nhiễm độc mạn tính); 0,3% người lao động có hàm lượng chì máu > 80 µg/dl (nhiễm độc cấp tính). Yếu tố liên quan: người có tuổi nghề 6 - 10 năm nguy cơ nhiễm độc chì cao hơn nhóm có tuổi nghề ≤ 5 năm 7,9 lần; có tuổi nghề ≥ 11 năm, nguy cơ nhiễm độc chì cao hơn người có tuổi nghề từ 6 - 10 năm 3,2 lần. Người lao động làm việc ở công đoạn đúc nhôm, cán nhôm và tạo hình có nguy cơ nhiễm độc chì 30% so với nguy cơ nhiễm độc chì của người làm ở công đoạn cô nhôm. Kết luận: tỷ lệ nhiễm độc chì 11,7% (đa số nhiễm độc mạn tính). Nguy cơ nhiễm độc chì tăng theo tuổi nghề và phụ thuộc vào công đoạn sản xuất của quy trình tái chế nhôm.

Abstract
Objectives: To describe the current status of lead poisoning in workers in Binhyen aluminum recycling village in Namdinh province and find out some related factors. Subjects and methods: A cross-sectional study was conducted from 2015 - 2016. There were 418 workers aged 18 - 60 years who were randomly selected to participate in the study. Using atomic absorption spectrometry (GFAAS) to determine the level of lead in the blood of workers. Assessment of poisoning status in accordance with Circular 15/2016/TT-BYT dated 15 May 2016 of Ministry of Health. Results: Lead poisoning: 88.3% of workers had blood lead content < 40 μg/dL (safety level); 11.4% of employees had blood lead content of 40 - < 80 μg/dL (chronic poisoning); 0.3% of workers had blood lead levels > 80 μg/dL (acute poisoning). Relevant factors: workers with 6 - 10 years of occupational exposured to lead poisoning were 7.9 times as high as those who with the 5 years of occupational age. Workers with over 11 years of occupational exposure to lead poisoning were 3.2 times as high as those who with the 6 - 10 years of occupational age. Workers working in aluminum casting, aluminum rolling and molding had a lead poisoning risk of only 30% compared with the risk of lead poisoning of aluminum workers. Conclusions: The results showed that lead poisoning was 11.7% (most of which were chronic poisoning). The risk of lead poisoning increases with age and depends on the manufacturing process of the aluminum recycling process.
